HIPAA Notice

This HIPAA Notice describes how ENDUREMINT LLC ("EndureMint," "we," "us," or "our") handles health-related information in connection with its role as a marketing and administrative services organization. EndureMint does not provide medical care, prescribe medication, or operate a pharmacy. Telehealth services are provided by independent licensed clinicians, and prescriptions are fulfilled by licensed pharmacies.

The Health Insurance Portability and Accountability Act of 1996 ("HIPAA") and its implementing regulations establish national standards for the protection of certain health information. HIPAA applies to "covered entities" (health plans, healthcare clearinghouses, and healthcare providers who transmit health information electronically) and their "business associates."

EndureMint is not a healthcare provider, health plan, or healthcare clearinghouse. EndureMint operates as a marketing and administrative services organization. Depending on the nature of the services EndureMint provides to or on behalf of its clinical and pharmacy partners, EndureMint may function as a business associate under HIPAA with respect to certain activities.

Where EndureMint acts as a business associate, it does so pursuant to a Business Associate Agreement (BAA) with the applicable covered entity. The BAA governs how EndureMint may use and disclose protected health information (PHI) received in that capacity.

Some of this information may constitute protected health information (PHI) under HIPAA when it is received or maintained on behalf of a covered entity partner.

EndureMint implements reasonable administrative, technical, and organizational safeguards designed to protect the confidentiality, integrity, and availability of health-related information, including:

Limiting access to health-related information to authorized personnel and systems on a need-to-know basis.

Using encryption and secure transmission protocols where appropriate.

Maintaining policies and procedures for information handling, incident response, and workforce training.

Entering into Business Associate Agreements with covered entity partners where required.

Where EndureMint acts as a business associate, it uses and discloses PHI only as permitted or required by the applicable BAA, HIPAA, and other applicable law. EndureMint does not use PHI for marketing purposes unrelated to the services described in the BAA without appropriate authorization.

Under HIPAA, you have certain rights regarding your protected health information, including the right to access, amend, and request an accounting of disclosures. Because EndureMint is not a covered entity, these rights are generally exercised through the covered entity (e.g., the independent clinician or pharmacy) that maintains your health records.

If you have questions about your HIPAA rights or wish to exercise them, please contact the clinician or pharmacy that provided your care. EndureMint will cooperate with covered entity partners in responding to valid HIPAA requests as required by applicable BAAs.

In the event of a breach of unsecured PHI, EndureMint will notify the applicable covered entity partner in accordance with the terms of the BAA and HIPAA breach notification requirements. The covered entity is responsible for notifying affected individuals as required by law.

EndureMint may update this HIPAA Notice from time to time. If material changes are made, the updated notice will be posted on the website with a revised Effective Date.
